The Surgeon’s Studio

“What are you thinking about at home?” Su Yun squatted down and fiddled with the patient’s chest tube.

He emptied the fresh blood that was drained out of the chest tube. As he measured the time, he bent a section of the chest tube to observe the speed of the drainage.

The chest tube was clear, and there was not much drainage. This could be confirmed.

Su Yun’s actions did not seem to be necessary, but Zheng Ren knew that he was eliminating the small details that might cause problems.

Many of the seemingly serious conditions were actually caused by the accumulation of small problems.

“Old Zhao, what about the B – list?” Su Yun fiddled with the chest tube as she asked.

“Professor Zhang,” Zhao Yunlong said.

Su Yun was also helpless.

It wasn’t as if professor Zhang was the best. Everyone’s level was different, and their ability to take risks and thoughts were different.

Some people were more radical, while others were more conservative.

Each had their own advantages and disadvantages.

As for professor Zhang, he would be a great teacher if he went to school. However, in such an extremely complicated situation, he might not be a good candidate for emergency treatment.

“Where is he?” Zheng Ren asked.

“We’ll be there soon,” “Boss … Zheng, can you help me keep an eye on him? I’ll go and urge the patient’s family again,” Zhao Yunlong said.

“Go on.” “Is the catheter room ready for the IAB?” Zheng Ren asked.

“Yes, director Zhang Lin is rushing over from home.” Zhao Yunlong said.

Zheng Ren nodded and did not continue speaking. Instead, he turned on the computer and started to read the patient’s medical record.

Zhao Yunlong’s description was too brief, only talking about the main points. He didn’t have time to go into detail about the rest.

Zheng Ren wanted to take a closer look at the patient’s condition.

The patient was 52 years old and had a family history of hypertension. His father died of cerebral hemorrhage. He had not had any symptoms before. Just as Zhao Yunlong had said, he had only come to the hospital two days ago because his chest was stuffy and his breathing had worsened.

In the medical record, the patient admitted that he had no history of dizziness.

Zheng Ren had his doubts about this. If there was dizziness, it would be the most reasonable for it to be accompanied by a hidden attack.

The patient’s medical history could only be described in this way. It could not represent everything. It could only be used as a reference, especially for a patient with such a complicated situation.

When he was admitted to the hospital, the CT scan of his head showed abnormalities, and he only had an MRI scan done a day later.

Zheng Ren could feel that the reason why the patient’s family did not sign the agreement for the IAB was because of family and financial factors.

Zheng Ren looked through the test sheets and various examination reports one by one.

He quickly gained a certain level of understanding and recognition of the patient’s condition. Su Yun looked at the medical records and then turned to look at the patient’s condition. Her neck was almost broken.

“Boss, the MRI scan shows that the patient has a serious case of smoke disease.” Su Yun commented when she saw Zheng Ren insert the MRI scan into the MRI scanner.

“Hmm, I’ll consider …” Just as Zheng Ren said that, the monitor’s alarm sounded loudly.

The beeping sound reverberated throughout the room.

Zheng Ren and Su Yun stood up immediately. The chair that Zheng Ren was sitting on fell to the ground with a loud bang.

“Get ready!” Zheng Ren roared in a low voice.

Su Yun was stunned for a moment. He subconsciously wanted to reject her. Without the signature of the patient’s family, he directly did IAB. Was he courting death?

However, he could hear the determination in Zheng Ren’s voice that could not be rejected.

Who cares! Su Yun made up her mind. Anyway, if the sky fell, the boss would be there to support it. At most, he would resign and work at Massachusetts General Hospital. That seemed like a good choice.

“The Helium used for the IAB machine and machine is ready!”

“IAPP catheter and puncture kit are ready!”

“0.9% NCL 500ml +5000U hepatilin, ready!”

“Pressure bag, maintain the pressure at 300 mmhcg?”

Su Yun asked.

“Sure.” Zheng Ren lifted the patient’s blanket, then opened the IAB puncture kit and prepared iodophor.

“Lidocaine,” Zheng Ren said in a deep voice as he put on his gloves.

Su Yun had already turned on the machine and pushed it to the side of the bed. She immediately unscrewed a plastic bottle of lidocaine and injected the syringe into the sterile bag.

Zheng Ren picked up the syringe and inserted the needle into the opening of the plastic bottle to extract lidocaine.

He did not dilute it. He grabbed a handful of iodophor with his right hand and disinfected the patient’s femoral artery. Then, he administered local anesthesia.

The puncture was successful, and the balloon catheter was inserted into the patient’s femoral artery. Zheng Ren’s actions were very fast. Su Yun had just placed the electrode piece on the patient’s chest when the two ECG monitoring systems started working at the same time.

By this time, Zheng Ren had already finished the operation and placed the IAPP tube to the descending aorta.

After opening the reverse – pulse pump, Zheng Ren handed the sensor that monitored the aortic pressure to Su Yun.

Su Yun connected the sensor to the main computer.

The flushing system was connected to the sensor, and the central cavity was connected to the pressure pipe.

Zheng Ren had basically completed the work he needed to do, and Su Yun was extremely busy.

After connecting the Helium pipeline, Su Yun looked at it a few times and confirmed that the Helium working pressure met the requirements.

“Let’s suture it. ” Su Yun said with certainty.

Zheng Ren nodded and began to stabilize the Helium pipeline.

Su Yun tested the various parameters on the main computer.

Zheng Ren knew that Su Yun was more professional than him in this aspect. Letting him do it was the most worry – Free way.

There was no need for him to participate at all.

As the iabi system began to work, Zheng Ren heaved a sigh of relief. The patient would be fine for the time being.

The machine fed the patient’s electrocardiography or blood pressure signals to the counter – pulse control device, so that the balloon pump moved in sync with the patient’s heartbeat in reverse.

The moment before the heart contracted, which was when the aorta opened, the balloon would be deflated to lower the end vasculated pressure in the aorta and reduce the left chamber from working. Reduce the afterload and reduce the oxygen consumption of the heart.

The moment before the heart relaxed, the balloon would be pumped with air to increase the coronary artery perfusion pressure during the dilative period and increase the oxygen supply of the heart muscle.

This could reduce the load on the left chamber and the heart. The balloon would quickly exhaust the air before the heart contracted and the aortic valve opened, causing the aorta to instantly deflate. At the same time, the left ventricular discharge resistance would be reduced, and the amount of heart blood would increase.

IAB had unparalleled significance in providing preoperative support and stabilizing the patient’s lifeblood.

With the activation of IAPP, the wildly screaming heart monitor gradually calmed down.

“Boss, the patient’s family members haven’t signed the form yet.” Su Yun adjusted the various parameters of the machine and said helplessly as she saw Zheng Ren packing up the things from IAB.

“I don’t have time.” Zheng Ren said,”by the time the patient’s family members finished signing, he would have already died.”

“Why hasn’t old Zhao returned yet?” Su Yun nagged, but her eyes were fixed on the monitor.

The emergency treatment this time could be considered a success, but the various values and indicators were still not ideal. The patient was still in a state of near death. As for when he could not make it, it would depend on fate.

“Boss …” Su Yun said with a frown. “I don’t think it’s right. The patient’s condition is too serious. It’s worse than I thought.”

Zheng Ren noticed this as well and was thinking about what to do next.
